This article covers common differential diagnosis of unilateral vocal fold paralysis in children and recommends a multidisciplinary systematic approach in the management of these children. It highlights the value of key appropriate investigations including stroboscopy, flexible naso-laryngoscopy, direct laryngoscopy, EMG and swallowing/voice/speech assessment. Moving further, it discusses the various management options, risks, benefits and outcomes (short- and long-term) of watchful monitoring, vocal fold injection therapy, medialisation thyroplasty and recurrent laryngeal nerve reinnervation. The senior author concludes by describing the surgical technique of recurrent laryngeal nerve reinnervation, the pre and postoperative care and the long-term outcome associated with this procedure. I personally liked the practical approach and the case examples highlighted in this article.
