The diagnosis of Ménière’s disease is clinical, and the authors tried to verify if endolymphatic hydrops (EH) could be demonstrated on MRI. In this prospective study, they analysed 3D3T FLAIR MRI images of 32 ears with definitive Ménière’s disease (DMD) and 11 patients with probable Ménière’s disease (PMD) approximately 21 days after the episode of Ménière’s. In addition, all patients underwent electrocochleography (EcoG), distortion-product otoacoustic emissions (DPOAEs) at 1 kHz between sitting and lying down positions, and videonystagmography (VNG) on an average 41 days after the episode. The MRI showed endolymphatic hydrops in 71.9% of DMD patients versus 27.2% PMD patients. The authors conclude that, in patients with DMD or PMD confirmed by audiovestibular testing, MRI images revealed endolymphatic hydrops more frequently in those with DMD, supporting the notion that endolymphatic pressure disturbances precede endolymphatic hydrops.
